Are you up for Financial Aid Verification?

You and/or your parents may be required to provide other documents along with a verification packet. These documents will be required of you and your parents, if you are a dependent student. You may be asked to provide additional documentation such as official tax transcripts, diploma, school transcripts, SNAPS, child support, SSI, SSA, etc. Verification packet may be obtained at www.www.livelytech.com.

Students (and parents if student is dependent) whose tax information wasn’t automatically pulled from the IRS import feature on their FAFSA will be required to provide an official Tax Transcript from the IRS or a signed copy of the 1040 plus applicable schedule(s).

Copies of tax filing forms from your tax preparer or copies of W-2s are NOT acceptable

How to obtain your Official Tax Transcript

  1. Request/download your Tax Transcript online at http://www.irs.gov/Individuals/Get-Transcript
  2. Call the IRS at 1-844-545-5640 (Press 1 for English and 1 again to speak to a representative) to have your tax transcripts faxed or to make an appointment at your local IRS office. You must be standing by the fax and verify that you have received all pages. Lively fax # 850-487-7430.
  3. Mail or fax Form 4506-T to the IRS at 855-821-0094

    – if you filed taxes and were unable to use the Direct Data Exchange to import your tax information.

  • Form 4506-T, 2022 Non-filing Transcript
    – If you did not work.
